About Us

Countryside Haven is a 30 bed all inclusive assisted living facility set in a quiet residential area in Palm Harbor.  Carin & Manny Pe were the previous owners of Countryside Haven ALF for 20+ years but have recently retired.  Kathy & Dave Coogan were chosen to be the new owner/operators and are grateful for the opportunity.  They have been in the ALF business for over 25 years and are excited to bring their teamwork and support to the current staff at Countryside.

At Countryside Haven ALF you will find a warm, friendly atmosphere where resident care is the primary focus.  Our goal is to provide for the health and wellbeing of each resident by making them part of our extended family.  We treat everyone as if they were our own grandparents and will always go the extra mile to ensure their safety and happiness.

 

The new owners and staff at Countryside Haven aspire to have an exceptional home.  They genuinely love to care for the elderly and return day after day with bright smiles and open hearts ready to give their best.

VISITATION POLICY DURING COVID-19 Facility Outbreak

***To date, we have not had any incidents of COVID-19 in our facility; however, in the event that someone is diagnosed, the following visitation policy will apply.

 

POLICY
The purpose of this policy is to allow residents to have visitation when the facility has 2 or more COVID positive cases. Everyone visiting the resident must follow procedures outlined. 
 
PROCEDURE
1. Visitation hours will be from 9am-9pm.
2. Visitation must be scheduled with the facility by phone or email prior to arrival to the community.
3. 2 people maximum to visit the resident at a time. 
4. Person visiting must:

  • Have their temperature checked at the door prior to entering the building.

  • Visitation questionnaire answered.

  • Review infection control education materials provided upon entry.

  • Visitor must wear face mask, and be free from COVID/Flu like symptoms.

  • Must wash/sanitize hands.

5. After visitation, the area will be disinfected by staff members.
6. Any 3rd party personnel providing care to residents MUST wear a mask, be screened at the door and MUST NOT have any COVID/Flu like symptoms prior to visiting resident to provide care.
